The team from Southend West Central Primary Care Network (PCN) has been out in Westcliff, offering patients the opportunity to get a basic healthcheck, as part of a project to help West Central patients manage their long-term conditions.
Patients identified as being at risk of cardiovascular disease across the five PCN practices were invited to attend an outreach clinic at Havens Community Hub on Wednesday 12 July, where members of the clinical team were on hand to carry out blood pressure, height and weight checks.
Clinical team leader Lionah Musengezi said, “It’s really important that patients who are at risk of developing cardiovascular problems get their blood pressure checked regularly. By holding this clinic we are able to take a reading and enter it on the patient’s GP record, so that it can be monitored more closely if necessary.”
The clinic proved extremely popular, with around 200 people taking up the invitation. One patient who attended said it was, “a very enjoyable experience, with very nice people and makes people feel relaxed.”
